All entries are subject to the rules of the American Kennel Club.
Entry Forms are included in the Premiums for each event. Please do not put credit card information on your entry form(s)! Entry forms must be archived for up to a year, and we don't wish to have your card number in our archive! The following items must appear on a cover sheet (a .pdf version is supplied above):
• Cardholder's name, address, city, state and zip code
• Phone Number and e-mail address
• Credit Card Number, type (Visa or MasterCard), credit card number and expiration date
• A written signature on the cover letter authorizing payment of entry fees.
• "CVV" Code: the last 3 numbers that appear in the signature area on the back of the card
• Total number of pages including the cover sheet
1. Faxed entries will be accepted up to the closing date and time. No Entry may be changed or cancelled except as provided in Chapter 11, Section 6 of the Rules Applying to Dog Shows.
2. Agility faxed entries are accepted under the rules of "first received," and cannot be processed in the first 24 hours after the opening date and time.
3. Both sides of the official AKC entry form must be included.
4. Entries that are conditional, unsigned, incomplete, without the back side of the entry form, or without credit card information will be considered invalid.
5. A written signature on the fax cover letter is required.
6. Entries of cardholders whose charges are declined by the bank card authorization center will not be processed.
7. Senders of illegible or incomplete transmissions will be notified by phone or e-mail if it has been provided.
8. Refunds made after closing are at the discretion of the show-giving club and will be made by the club.
9. Fax fee is $3.50 per dog in addition to entry fees and is non-refundable.
10. You may wish to confirm that your fax was received! Proof of transmission is not proof that your entry has been successfully received.
